Services: Cardiac & Pulmonary Disease Management Program
The Nursing and Therapy staff at Home Health Care Professionals and Hospice Inc. work in conjunction with your primary physician and medical specialists to develop a Plan of Care. The goal of our cardiac and pulmonary disease management program is to lessen the likelihood that you will need to be hospitalized, promote physical strength and endurance and give you a better understanding of your illness and how to manage it.
Cardiac Pulmonary Disease management programs may include:
- Counseling and Education so the patient can understand and manage the disease process
- Evaluation by Physical and/or Occupational therapy to being an exercise program if appropriate
- Education on nutrition
- Helping the patient modify risk factors such as high blood pressure, smoking, high blood cholesterol, physical inactivity, obesity and diabetes.
- Supplying information on physical limitations
- Lending emotional support
- Education on appropriate use of prescribed medications
Cardiac and Pulmonary Disease may include:
- Heart Attack
- Heart Failure
- COPD
- Emphysema
- Coronary Artery Disease
